description: Objective: To examine the extent of adoption and barriers to the implementation of a structured fall prevention program among clinical nurses and nursing leadership on five adult medical units in which the fall rate exceeds national benchmarks. Design: A mixed methods design was used. The “Diffusion of Innovations in Healthcare” (Titler & Everett, 2001) was used to evaluate the rate and extent of adoption of the fall prevention program. Proctor’s implementation research framework (2010) which included eight implementation outcomes was used to measure if successful implementation was achieved for assessing outcomes. Two author-developed surveys were completed evaluating nurses’ and nurse directors’ beliefs and knowledge about patient falls and the the structured falls program. The data was collected by emailing a link through an intranet site, resulting in a sample size of 108 clinical nurses and 5 nurse directors. Qualitative data was collected through two focus groups, which included ten clinical nurses and five nurse directors. The data was studied through a thematic framework analysis. Findings: Findings suggest clinical nurses perceive the LEAF Program is important as evidenced by the summative scores on the survey Perceptions Subscale, but do not always follow the LEAF program consistently. A correlation was found between the clinical nurses’ perception of the effectiveness of the LEAF program and the adherence and adoption of the program. Many participants believed that their own nursing knowledge and intuition was sufficient to address the problem of falls. A substantial number of participants were not familiar with the components of the LEAF program, indicating a deficit in LEAF knowledge. Conclusion: Re-education of the LEAF program and instituting decision support is likely to have a positive impact on successful implementation of the LEAF program.
